Form Instructions
Lookup your students form
Enter your student's Londonderry School ID# and your email address. The student ID# is a 7 digit number beginning with a 9. It can be found in the Aspen Parent Portal. If you are unable to access your Aspen Parent Portal please contact your student's school main office.
Check the acknowledgement box
Click "Search".
Select your forms
You will see your student's available request for example: "High School Parent Roster 2024 forms."
Click "Start Submission"
Begin forms
Enter your first and last name. Your email should be prefilled, if not enter it.
Check the acknowledgement box.
Click "Next"
Verify student name
Your student's name should be prefilled based on our official record of your student. Please do not change it to a nickname.
Click "Start"
Complete online forms
There are a series of forms to fill out for your student. On each form you can click "Submit" to move to the next form or "Save for Later" if you wish to complete the form at a later date.
Once you start the form process you will receive an email for each student with a link to return to complete the form.
Emails will come from "alerts@scriptapps.com"
You and your student will need to digitally sign the Handbook Policies form.
Once you have submitted all the forms you will be prompted to download a PDF of your completed forms. You can save the PDF for your records.
